Yes, the employee made a mistake, but who put them in a position to make that mistake? The senior DBA? Who put the senior in that position? Ultimately everything comes down to the manager, otherwise, what the hell are you there for?

I have to disagree with this to a point. You appoint people and give them responsibility, and that means owning their own mistakes. The senior DBA I'd say had the responsibility to ensure the junior was up to the task, not the manager. This is also what Change Control is to protect against: mistakes being made, and when they are to provide a) a route out of them, and b) a clear indication of where responsibility lies.

Saying that what the manager did is definitely going above and beyond, and all credit to him, but he did it at least partly for his own selfish reasons (keeping his team together).

I'm presuming the account resides at the location, just the customer name and other details change?

Does rather sound like a system prone to many potential pitfalls to me..?!

I still use notepad pretty much every day in my job - have yet to find a better way of stripping all formatting and hyperlinks off text in one swift move for moving between various applications,

I do the same, as when you copy/paste text from modern web apps it often comes with extra unnecessary formatting. I call it 'washing' the data.

I've got an ISO image of that somewhere. It was available from the old Microsoft TechNet library, and one quiet day I downloaded it, installed it into a VM, played with it for 10 minutes enjoying the nostalgia, then shut it down and deleted the VM!

In my first IT job I set up the BSoD screensaver on the team NT server. Very pleased with myself and used to get amused comments from other IT techies... until the day it BSoDed for real!
